From what I know, it's either the FDS port of Super Mario Bros. 2 (The Lost Levels) or Super Mario Bros. with level select.

That cover is quite common. Almost as much as LF36.

Its definitely Super Mario Bros. 2 Japan with the label pasted on upside down. Ive never seen a multi cart use that label and the SMB with level select does not use this label either, Ive never encountered them at least and Ive had about 20 of these pirate SMB2J carts come my way the past 15 years....

Proof that it is not always SMB2j:
(http://www.hapshack.com/images/XyzHb.jpg) (one of the lost worlds, as it is SMB256W rom hack)

You may not see the cart clearly, but it has the exact same cover as the one pictured. Not any difference.

I suppose it's SMB1 with 2J's graphics, as shown by the clouds. The mushrooms weren't updated since they were replaced by the cloud platforms in 2J, and it wasn't until All Night Nippon where the stems were given appropriate upgrades.
